Toothache, or odontogenic pain, is one of the most common reasons patients come to the surgery. It tends to strike on a Friday evening, over the weekend, just before a holiday \u2014 and, worst of all, during one. Joking aside, patients describe toothache as unbearable, one of the worst kinds of pain they have experienced.<\/p>\n
The most common cause of odontogenic pain is a condition we call pulpitis or, as patients know it better, inflammation of the dental nerve. It can be caused by untreated decay, a filling close to the pulp (the \u2018nerve\u2019), dental trauma or an activated periapical infection. Patients usually describe this kind of pain as throbbing. In these situations the solution is endodontic treatment, i.e. treating the \u2018dental nerve\u2019. Until you can see a dentist, painkillers are the first line of relief. Among them, ibuprofen is the drug of choice for odontogenic pain. For children, pregnant or breastfeeding women, and patients for whom ibuprofen is contraindicated, paracetamol-based medicines are used.<\/p>\n
Besides throbbing pain, discomfort can also be triggered by certain irritants. Patients often complain of pain with cold. In most cases the reason is gum recession, which exposes the dentine. Dentine is the part of the tooth that is normally covered by the gum. There are several reasons why dentine becomes exposed, but one of the more common ones is incorrect brushing. Good brushing technique matters more than the hardness or type of toothbrush.<\/p>\n
Pain can also be caused by changes in the periodontal tissue around a tooth. Poorly fitting restorations can lead to food build-up, make cleaning harder and result in inflammation. When a third molar, or wisdom tooth, is coming through, food can get trapped under the flap of gum covering it. The pain is then localised and throbbing, and it can also make it difficult to open the mouth.<\/p>\n
